CHIP of Greater Richmond
CHIP is a voluntary and free program that provides case management, home visitation services and mental health evaluations to low-income parents and children. They work to make a difference in the lives of children through an innovative team approach to home-centered health education. By providing holistic services to high-risk families with young children in the Metro Richmond area, CHIP works in partnership with these families to improve the health and well-being of their children.
Programs Supported by the Community Action Fund:
CHIP works with parents to improve the health and wellbeing of their children through family case management. The program uses teams that visit families in their homes and develop relationships by building on the positive and not focusing on the negative. Each team consists of a Family Intervention Specialist (a specially trained lay home visitor or FIS) and a community health Registered Nurse. A Licensed Clinical Social Worker (LCSW) also provides mental health assessments and counseling as needed. The FIS instructs and models life and parenting skills, assists in learning to use community resources, and helps parents learn ways to solve life’s problems. The RN provides critical health and developmental assessments, teaches healthy habits and positive parenting, and coordinates the family’s health care.
CHIP of Petersburg offers a home visitation program for low-income, high-risk children through the Petersburg Health Department. The program is designed to improve the health of children in low-income families, increase family self-sufficiency, and reduce barriers to the delivery of comprehensive services for low-income families. In order to meet these goals, CHIP’s multidisciplinary staff works with families and community agencies to help families gain access to medical care. CHIP’s model is one of collaboration through family support services, care coordination services, and primary health care services.
